AI medical bill audit: itemize, check codes, dispute in writing
A careful AI-assisted workflow to request an itemized medical bill, flag coding issues, cross-check CMS tools, ask about financial assistance, and send a dispute letter.
Hospital and clinic statements often show a summary total. Your leverage starts when you have itemized CPT/HCPCS codes. AI can help flag duplicates and odd units — but it is not a medical coder or attorney, and you must look up rates yourself.
Educational only. Not medical, legal, or billing advice. One widely reported first-person account described a very large bill reduced after coding issues were raised; outcomes vary. Calendar every insurer and provider deadline.
Step 1 — Request itemization
Ask billing for a complete itemized bill with CPT/HCPCS, revenue codes, units, and charges. Request the financial assistance policy at the same time. Ask that collections be held during review.
Step 2 — Redact, then audit
Remove name, DOB, member ID, SSN, and account numbers before using any AI tool.
Here is a redacted itemized bill with CPT/HCPCS and charges [PASTE]. Flag duplicates, possible unbundling / mutually exclusive pairs, implausible units, and inpatient vs outpatient mismatches. Cite codes and plain-English reasons. Do not invent codes. Mark uncertainty. This is educational, not a coding determination.
Run a second model on the first model's flags.
Step 3 — Look up, don't invent
Use CMS Physician Fee Schedule and hospital price-transparency tools for educational comparisons. Paste your lookup results back into the chat if you want help wording a question — never let the model invent Medicare allowed amounts.
Step 4 — Assistance + dispute
Nonprofit hospitals generally must publish financial assistance policies. Submit if you may qualify. Then send a written dispute listing each issue, attaching lookups, and requesting a written response.
Verify
Every flagged line exists on the real bill. Settlements are in writing with a zero-balance statement. You never pasted unredacted PHI into a public model.
FAQ
What is an itemized bill? A line-by-line list of every charge with billing codes (CPT/HCPCS, revenue codes) and dates. Hospitals generally provide it on request; it is the starting point for any audit.
Should I paste medical records into an AI tool? Redact your name, dates of birth, member IDs and account numbers first. The audit works on codes, descriptions and amounts.
Does AI know Medicare rates? Not reliably. Look up rates yourself on official CMS tools and use the AI to compare and draft letters.
